Follow this checklist to prepare for the U.S. Soccer Foundation's Grants cycle.

  4. Gather your required documentation

    Footprint Field Grants Required Documentation:

    • 501 (c)3 Tax Exempt Form (unless you are a college or university, municipality, public school, or a sovereign tribal nation)
    • W-9
    • IRS Form 990 or most recent audited financial statement (if you don’t have either, most recent bank statement)
    • Full Budget (list full expenses for field project & include a separate column that will list the products and services that specifically need funding assistance from the US Soccer Foundation)
    • Site Plan – this is a professional outline of the field space and MUST include all amenities and measurements (lighting, benches, goals, etc.). To get expert assistance with your plan from CHA Sports please click here to submit your inquiry.
    • Land Ownership or Land Use Documentation - You must prove that you own the land where your project will take place (ex: title or deed) or prove that you have a minimum of 10 year lease on the land (ex: land lease or land use agreement).

    • Right to Use Lighted Fields – If your project includes the installation of lights you are required to submit written documentation proving that you have secured approval from the community where your project resides.
